Our school nurse is on-site to help keep our students and staff safe and healthy. The district's nurse practitioner also provides physical exams to students at no cost. Before performing a physical exam, legal guardians must complete a parent permission form. Everyone has to sign this form when they register their child for school. Please check with the nurse if you have any questions or concerns about this form. The form also grants the nurse permission to discuss health information with your child's physician if medically necessary.
